New Patient Registration

Welcome to Spallone Family Dentistry & Associates!  We are delighted that you have selected our practice to serve your dental needs.  Before you begin your New Patient Registration, please take a moment to read the following instructions:

When you access our website at spallonefamilydentistry.com, click on the tab “New Patient Forms” and scroll down to the paragraph titled “New Patient Registration.” Please read the instructions before clicking on the “Register Here” button. When you click on the “Register Here” button, a page will open asking you to verify your identity. Please put in your first name and last name and your appointment date if you have one.  If you do not have an appointment scheduled, you may put in the current date to proceed to fill out the forms but please note that this DOES NOT make an appointment for you. You must call the office to schedule an appointment. You will see two forms with two separate links, one for New Patient Registration All, and a second link for Patient Medical History. Please complete both forms. Note that upon completion of the first form and clicking submit, your screen will show “Awesome! New Patient Registration All Submitted” and it will have a button to click on for the Next Form. Click on Next Form and complete the medical history. Upon completion of that form, your screen will again show “Awesome! Patient Medical History submitted.” Please click on the button “Done” to finalize the submission of your forms to our office at which time your screen will state “You’re all done!” You will not receive any other form of confirmation, but rest assured your form has been submitted. If you would like to verify that we have received your form, or if you need to schedule an appointment, please feel free to call our office at 772-567-5100 and we will be happy to confirm that for you.
